By submitting this application to BJA NTTAC, I understand that upon approval of this application for TTA, the requestor agrees to keep BJA NTTAC informed of any circumstances that may impact the delivery of the TTA, including changes in the date of the event, event cancellation, or difficulties communicating with the assigned TTA provider.

Sent request for TA assessment call on 7.9.13. Send assessment form as well and asked them to fill it out. Have had no response. Sent second e-mail on 7.16.13 requesting a conference call. Attached assessment form again and asked him to fill it out. Sent third and final request for an assessment call on 7.23.13. In e-mail I stated that if I didn't hear from requester by 9:00am on 7.30.13 that the request would be closed. Did not receive communication to schedule a conference call and did not receive assessment form by deadline date that was sent. E-mail was sent to requester on 7.31.13 advising him that request was being closed due to inactivity. He was invited to apply again in the future when ready and link to TTA application was sent.
